Come to the HealthLinc Valparaiso Diabetic Clinic this Friday, May 10th - 9am-12noon! "While diabetes and prediabetes occur in people of all ages & races, some groups have a higher risk for developing the disease than others. Diabetes is more common in African Americans, Latinos, Native Americans & Asian Americans/Pacific Islanders, as well as the aged population. The National Committee for Quality Assurance (NCQA) has endorsed the Valparaiso clinic as a Patient-Centered Medical Home provider. “The NCQA Patient-Centered Medical Home is a model of 21st century primary care that combines access, teamwork & technology to deliver quality care & improve health”-- NCQA Pres. Margaret E. O’Kane. “NCQA’s PCMH Recognition shows that HealthLinc’s Valpo's clinic has the tools, systems, & resources to provide their patients with the right care at the right time.” Health Care, Medical Services, Medical Center, Internal Medicine, Medical Dental, Health Services, Social Security Card

Camille Cooke, community outreach manager for HealthLinc, said ADA provides the materials while the staff at HealthLinc provides the services during the annual event — annually slated for Presidents Day when children are not in school. Cooke said HealthLinc provides dental services year-round, but the annual Give Kids a Smile Day is way to attract families with no insurance to introduce them to their services. Hats off to our Valpo staff for recent PCMH recognition! Hope you saved us some celebratory left-overs!

Over the past several months, the staff of the Valparaiso clinic pulled together and did everything needed to achieve PCMH designation. Now they deserve to celebrate! Congratulations HealthLinc Valpo!!! The National Committee for Quality Assurance (NQCA) has awarded the HealthLinc Community Health Center in Valparaiso, IN with Level 2 PCMH Recognition (Patient-Centered Medical Home). It wasn't easy, but as a team, we came together to develop the policies and procedures that were needed to get the Recognition, and HealthLinc Valpo's Providers and staff worked to implement the changes, and deserve our applaud!
